If UPPER and lower are both compatible, I guess I don't care. Yes, the
upper-case usage is a vestige of SGML.
Oh, wait. It would require us to fix every single document we've
written so far. This is not a decision to take lightly or do on a whim
because it seems easier.

Mr. Flip-Flop here. I suppose we could add lowercase definitions to the
included entities file, right? Of course, I came up with this
earth-shattering idea after hitting "Send." :-)
