In 2.2.3 we addressed that by detecting the virtinst version
and disabling certain features (like disk and net drivers). It looks
like we need to add --autostart to that list.

This a definite bug, and I think at least one other person reported
it. The issue is the version of python-virtinst that is available to
RHEL5 (and clones) is quite limited in the command line flags it
accepts. In 2.2.3 we addressed that by detecting the virtinst version
and disabling certain features (like disk and net drivers). It looks
like we need to add --autostart to that list.
The 404 would seem to be some kind of apache/cobbler/wsgi
misconfiguration, but isn't what ultimately kills the install.
