I wanted to add the ability to import VMware ESX 4 update 1 with
cobbler import.
I got this running by manually creating a pxelinux.cfg/... file based
on this documentation

Mine actually looks like this:
kernel ESXi4/mboot.c32
append ESXi4/vmkboot.gz --- ESXi4/vmkernel.gz ---
ESXi4/sys.vgz --- ESXi4/cim.vgz --- ESXi4/ienviron.tgz ---
ESXi4/image.tgz --- ESXi4/install.tgz --- ESXi4/oem.tgz
This worked fine for the installations I needed to do.
I never got it running from cobbler because cobbler has quite strict
requirements on the OS: it needs both a kernel and an initrd And based
on this you don't have an initrd at all.

Where did you get those two files from?
What were they called in the original distribution?
