I had this problem some time ago with a production system, but i could
solve it somehow... i can't remember exactly what I did.
Now, I've tried to reproduce the problem in a clean debian wheezy
installation in a virtual machine and found that is working perfecly.
The script called from pam_exec can read password from stdin regardless
the existence of the user.
As you said it should be a configuration/stacking issue.
I'm sorry for the inconvenience caused.
