Looks like it is definitely a freetype problem. I suggest filing a freetype bug.
I tried LD_LIBRARY_PATH both http://ftp.uk.debian.org/debian/pool/main/f/freetype/libfreetype6_2.4.9-1.1_... http://ftp.uk.debian.org/debian/pool/main/f/freetype/libfreetype6_2.4.9-1.1_... then http://koji.fedoraproject.org/koji/buildinfo?buildID=310621
and indeed I got the 6th glyph and only the 6th glypth wrong in all 4 versions. then I just did
git checkout VER-2-4-11 ./autogen.sh CFLAGS=-m32 LDFLAGS=-m32 ./configure
and LD_LIB... still got the partially reversed-video look like I had with rpm-driven built 32-bit freetype.
So we agree perfectly with freetype 2.4.9 32-bit or 64-bit outcome, but not 2.4.11 32-bit outcome. How did you build yours?
This does not rule out cairo passing indetermined parameters to freetype (like the signed/unsigned load_flags which are ~mask'ed, I mentioned earlier) though.
