I could not find a set of comprehensive installation instructions. Does
such a document exist?

You should put absolute path there if the location is
config/mysql-connection.php, by default the php-virt-control looks to
/etc/php-virt-control for the file specified, i.e. it's trying to look
for /etc/php-virt-control/mysql-connection.php in this case IMHO.
Hope this helps and if not, I'll try to reproduce the behavior to
provide better help.
