This might be a good idea to enhance cooperation.
I'm actually
looking at O3's DPACS (based on dcm4che/chee as well); if the manual
is to be believed, it would continue to run without imageio, but
without the ability to do image compression/decompression. On the plus
side the company developing it has ISO certification:

but on the other hand they seem to make life a bit harder for outside
developers by doing periodic code drops rather than opening up their
internal repos...
After a quick look I did not found any licensing statement. If you
found one, I'd be happy to know and would list it amongst Debian Med
prospective packages.
