One should use detach and then remove. If Destroy action is used then manual cleanup should do it.

That's correct. However there are times when you can't do this, or perhaps the storage you got to use wasn't "cleaned" before being allocated to you. We are still missing the flow where we report the "dirty" storage and then have a process to "clean" it for us.

The 'remove' option is always grayed out. Are there any steps I should do after adding the storage to make it available?
iirc, because the LUN is not clean (detected as being used by a RHEV PV already).

RHEV doesn't show a "used LUN". when you remove a storage domain, it is formatted, and can be re-used. when you destroy - assumption is no access to storage to format it, and you need to manually format it to re-use it in RHEV. (dd to begining of the LUN should solve this).
probably should show these LUNs in gray out at least in the UI - I think we have an AI on that - I still need to move them to upstream bugzilla.
