In Task Manager Settings - right click the Task Manager - it's possible to set: A. Grouping to: (1) Do not group, (2) Manually, (3) By Program Name (default) B. Sorting to: (1) Do Not Sort, (2) Alphabetically (default), (3) Manually, (4) By Desktop.
When two instances of an application - eg 2x Konsole - are opened, with the default (A3+B2) settings, they will appear in the Task Manager as one button with a little "2" at the right. Clicking on this button, pops up a box where you can choose between the two consoles.
Next, in Task Manger Settings leave Grouping the same and change Sorting to Manually (A3+B3). Now, no box pops up when you click the 'Konsole-button'. It's not possible to choose between the two consoles.
Is this a bug or just by design?

Sounds like clearly a bug. Please file at bugs.kde.org.

FYI: With the latest update - 4.2.0 - this problem is solved.
