Hi,
the managed entry plugin is automatically creating a user private group
entry when a user entry is created (see [1] and [2]).
To help diagnose, you can enable the plugin logging by setting
nsslapd-errorlog-level to <your level> + 65536 as described here [3].
This should help understand why the plugin rejects the add operation.
