Is theres any template for feedback form available somewhere for ambassadors use?
I believe there are types of contents that are useful for FedoraProject and those that are not. A random survey without goals I dont think it'll be useful.

lets take for example ..
new users .. they may have initial views on Fedora .. what they think is hard and what they think is easy in how Fedora handle stuff ..
a person who have been using Fedora for some time or so may have different opinion about stuff .. problems they had experienced with fedora , things they love in fedora , what they think can be improved etc
those who left fedora for other distros will have some sort of reason behind it... what causes them to leave, what they like in other distros and what they hate in Fedora ..
Subjective questions are not easy to deal with .. and there need to be some sort of organized customer feedback for us to get the feedbacks and analyze it to find ways to improve Fedora .. we need a set or standardized questions for collecting feedbacks ..
I dont know much what information considered useful to FedoraProject and what information are not ... perhaps someone more experienced can enlighten me ...

The idea is good..but having a very clear cut path often destructs the flexibility. According to me there IS some need to organised feedback but not in a too organised way.
For example we can outline what we want to ask 'em.But having a printed form is pointless.
What my experience says , talking to someone for a reasonable time often brings out feedback which are quite unexpected and which are seen from a entirely different point of view.
I fear having a too much standardised question set is not going to help much.. We may discuss and gather a few questions which are to be asked. But at the end it should be upto that person who is collecting feedback.
And I would like to object to the term *customer feedback*.. :-). I don't think those who are using Fedora are customers. User feedback is a more appropriate term.

yup .. the outline is what most important in all ... having a good outline is good enough for me :)

a standardized set of question would be useful in large scale feedback .. its easier to analyse lots of data if its standardized ... for small scale feedback, I agree that having a good outline on what to ask is more suitable ... what FedoraProject consider useful info and what not ..
also, having a template/sample form for feedback doesnt hurt .. it helps to reduce unnecessary brain ache because of trying to find idea on what to put in a feedback form ..
And I would like to object to the term *customer feedback*.. :-). I don't think those who are using Fedora are customers. User feedback is a more appropriate term.

Would really depend on what you intend to capture. For example, you can target UserCategories:
* Desktop User (new or migrating from Windows) * Desktop User (migrating from other Linux OS) * Server User (System/Network Administrators) * Specialised Desktop User (Graphics, Animation, Sound etc)
and there on drill down to:
* Which bits were difficult to handle during installation * Which bits of the regular/daily workflow is difficult/different * How often do they update their boxes - do they use Auto Updation * Are they able to use the complete power of their hardware (eg Display with max possible resolution) * Which hardware bits did not get detected and/or configured properly * Were there data loss due to data formats being locked-in
