[PATCH conductor] CSV export for instances

I am not sure where link to this action should be, it is acessible only from /instances.csv.

aeolus-image push failure with rhev3 api

I'm attempting to push an image using `aeolus-image push --provider rhevm --id 387aa19c-4ebf-4717-9c98-896c28e2ffff`. I see the template get created in the RHEVM export domain and begin to get imported into the datacenter. While the template in the datacenter is still in a "locked" state (being imported from the export domain in RHEV) aeolus-image fails. Below are the details, any advice is appreciated.
I'm not sure how/if FedoraBuilder.py or ImageWarehouse.py are determining when a template has been imported from a export domain or if it checks if the template is still locked and keeps waiting. It appears it just has a set time that it checks and if the template is locked then it fails. I hope the answer isn't "buy faster storage". :)

formtastic implementation

This weekend I made myself familiar with the conductor forms
implementation by switching the user form to use formtastic
https://github.com/justinfrench/formtastic . As a runtime example,
https://openshift.redhat.com/app/user/new/express , you can see the
dom structure that is created via formtastic. rubygem-formtastic is
already in fedora and EPEL.

If the team is intrested in using formtastic for the application
forms, I'm happy to go through the existing forms and port them.
